Kidney Transplant

Patients are encouraged to learn about the kidney transplant process. Those interested in becoming a kidney transplant recipient will be referred to our dedicated Transplant Coordination team. NANI's Transplant Coordinators will then work closely with each patient to refer them to their preferred transplant centers. Once a patient is evaluated and deemed a candidate by the transplant center, they will be placed on the wait list. The NANI Transplant Coordinator will continue to track each patient's status during the wait list and post-transplant periods.

Our Process

NANI is committed to providing exceptional care and support to our kidney patients throughout their transplant journey. We have established partnerships with renowned transplant centers across Illinois, Indiana, Iowa, Ohio, Wisconsin, and New Jersey to ensure that our patients have access to the highest quality transplant services.

Referral

Patients interested in becoming a kidney transplant recipient will be referred by their NANI Provider to our dedicated Transplant Coordination team. A Transplant Coordinator will then contact the patient within 5 business days. During the initial conversation with one of our Transplant Coordinators, a review of the following information will be conducted:

  • Current insurance coverage
  • Medical history
  • Local and out-of-state transplant centers
  • Living Donor options

After this initial conversation, the Transplant Coordinator will submit a referral to the patient's preferred transplant center(s) and communicate the status of the referral as updates are received. Once the referral is accepted, the patient will be scheduled for an evaluation exam with the transplant center to determine if they are a candidate for transplant.

Post Transplant

Congratulations! After a successful kidney transplant surgery, the patient will remain in the care of the transplant center for at least the first year following surgery. The NANI Transplant Coordinator will continue to serve as a contact for the patient and will inform the NANI Provider of the successful transplant. The patient's NANI Provider will plan to see the patient back in their office approximately 6 months – 1 year after the transplant surgery.
