Even though we are contracted with most insurance providers, it is best to confirm your coverage with your insurance company directly. NANI physician services are covered by both Medicare and Medicaid.
Check In with Ease
Please arrive 15 minutes early to complete any necessary paperwork before the appointment begins. The following information may be helpful as you prepare for your appointment:
- Photo ID
- Insurance card
- Medication bottles or a list of medications with dosage and frequency
- Primary Care Provider’s referral or authorization, if it is required by your insurance
- Appropriate legal documents, like Advance Directives or Power of Attorney for Healthcare if you have them
- Primary Care physician’s name and address
- Method of payment for co-pay, which is due at the time of service.
